Each Lasix tablet contains furosemide as its sole active ingredient. Furosemide belongs to the loop diuretic class, named for its site of action in the thick ascending limb of the loop of Henle in the kidney. It blocks sodium and chloride reabsorption, increasing the amount of sodium, chloride, and water removed in urine. This reduces excess fluid and can lower blood pressure, but it can also cause dehydration or clinically important electrolyte losses if the dose is too strong for an individual patient.
Common Uses
Clinicians prescribe furosemide tablets to reduce edema, or fluid buildup, associated with conditions such as congestive heart failure, kidney disease, and liver disease with ascites. Less swelling can make movement more comfortable and may relieve symptoms related to fluid congestion, but furosemide treats the excess fluid rather than curing the underlying heart, kidney, or liver disorder.
Furosemide is also used to manage hypertension, often together with other blood pressure medicines. General Dosing and Warnings
How and When to Take Lasix
Furosemide dosing is individualized according to the condition being treated, kidney function, other medicines, fluid status, and response. For adult edema, labeling describes an initial oral dose of 20 mg to 80 mg given once. If needed, a clinician may repeat that dose or increase it by 20 mg to 40 mg no sooner than six to eight hours after the previous dose until the desired diuretic response is obtained. Maintenance treatment may be given once or twice daily or on selected days. For adult hypertension, a commonly described initial total is 80 mg daily, divided as 40 mg twice daily, although a prescriber may choose another regimen. These are general examples, not instructions to change a prescribed dose, and the available 40 mg and 100 mg tablet strengths do not determine the right dose for an individual.
Pediatric oral dosing is weight-based. Labeling describes an initial dose of 2 mg/kg, with increases of 1 mg/kg or 2 mg/kg no sooner than six to eight hours after the previous dose if necessary; doses above 6 mg/kg are not recommended. A pediatric prescriber must calculate and monitor the dose. Older adults and people with impaired kidney function may also need especially careful dose selection and monitoring.
Oral furosemide generally begins increasing urine output within about one hour, reaches its strongest effect during the first few hours, and may act for roughly six to eight hours. It is usually taken in the morning. If a second daily dose is prescribed, taking it in the early afternoon may reduce nighttime trips to the bathroom. Take Lasix exactly as directed, with or without food, and use food consistently if it helps prevent stomach upset. Swallow the dose with water while following any fluid limit or hydration plan provided by your clinician.
Many prescribers ask patients to track blood pressure and body weight. Weighing at the same time each day, under similar conditions, makes changes easier to interpret. Report rapid weight gain, unexpectedly rapid weight loss, worsening swelling, shortness of breath, or a major change in urine output. Periodic blood tests may be needed to check potassium, sodium, magnesium, kidney function, and other measures affected by diuresis.
Missed Dose
If you miss a dose of Lasix, take it when you remember unless it is already late in the day or close to the time of your next scheduled dose. In that situation, skip the missed dose and resume your normal schedule. Do not take two doses together to make up for a missed one, since doubling the dose can increase urination, dehydration, low blood pressure, and electrolyte imbalance. Ask your prescriber or pharmacist for advice if your dosing schedule is unusual or you are uncertain what to do.
Suspected Overdose
An overdose of furosemide can cause profound diuresis, dehydration, low blood pressure, dizziness, fainting, weakness, confusion, and muscle cramps related to fluid and electrolyte loss. Severe cases can disturb heart rhythm or kidney function. If an overdose is suspected in the United States, contact Poison Control at 1-800-222-1222 promptly. Call 911 for collapse, seizure, trouble breathing, inability to wake, or other immediately life-threatening symptoms. Do not wait for symptoms to develop or try to induce vomiting.
Precautions
Allergy Warnings
Do not take Lasix if you have had a serious allergic reaction to furosemide or any ingredient in the tablet. Furosemide is chemically related to sulfonamides, and labeling warns that patients allergic to sulfonamides may also react to it. A history of a sulfa drug reaction does not predict the same response in every patient, but the specific drug and type of reaction should be discussed with a prescriber before treatment. Seek emergency care for facial or throat swelling, trouble breathing, or a rapidly spreading blistering rash.
Health Condition Warnings
Furosemide is contraindicated in anuria, meaning an inability to produce urine. It requires careful use in people with kidney impairment because excessive fluid loss can reduce kidney perfusion and worsen laboratory measures of kidney function. People with cirrhosis or other severe liver disease can be especially vulnerable to abrupt fluid and electrolyte shifts. Gout may worsen if uric acid rises, while low blood pressure, dehydration, or an existing sodium, potassium, or magnesium imbalance can become more severe. A clinician may correct fluid or electrolyte depletion before starting or increasing treatment.
Furosemide can raise blood glucose and affect urine glucose testing, so people with diabetes may need closer monitoring and possible adjustment of their diabetes plan. Tell the prescriber about urinary obstruction, difficulty emptying the bladder, lupus, hearing problems, or recent severe vomiting or diarrhea. Excessive sweating, fever, or reduced fluid intake can also change how a usual dose affects hydration and blood pressure.
Available human pregnancy data are insufficient to establish risk, and use during pregnancy requires a clinician to weigh potential benefit against fetal risk. Furosemide passes into breast milk and, as a potent diuretic, may reduce milk production, so the nursing infant and milk supply may need monitoring. Pediatric use requires strict weight-based dosing, and premature infants have additional kidney-related risks. Furosemide may cause dizziness, lightheadedness, or blurred vision, particularly when treatment begins or the dose changes. Stand up gradually and assess your response before driving or operating machinery.
Side Effects of Lasix
Common effects reflect furosemide's diuretic action. They include increased urination, thirst, dizziness or lightheadedness when standing, headache, mild digestive upset, and muscle cramps. These effects may be more noticeable after a dose increase. Sun sensitivity can occur, so protective clothing and broad-spectrum sunscreen may be useful during outdoor exposure. Alcohol can make dizziness and low blood pressure more pronounced.
Furosemide can lower potassium, sodium, magnesium, and calcium or produce excessive fluid loss. Possible warning signs include dry mouth, intense thirst, reduced urination, unusual weakness, drowsiness, confusion, nausea, vomiting, muscle pain or cramps, and a fast or irregular heartbeat. Contact a clinician promptly if these occur. Patients receiving long-term treatment commonly need periodic assessment of electrolytes, kidney function, blood pressure, weight, and symptoms rather than relying on symptoms alone.
Prompt medical attention is also warranted for ringing in the ears, hearing loss, fainting, severe abdominal pain, unusual bruising or bleeding, yellowing of the skin or eyes, or a widespread rash. Hearing-related toxicity is more likely with high doses, rapid intravenous administration, severe kidney impairment, or other medicines that affect hearing, although it can still require attention in a patient taking tablets. Severe allergic reactions and blistering skin reactions require emergency care.
Medicine Interactions
Furosemide has clinically meaningful interactions with prescription medicines, over-the-counter products, and supplements. Nonsteroidal anti-inflammatory drugs such as ibuprofen and naproxen can reduce its diuretic and blood-pressure-lowering effects and may increase kidney risk, particularly in dehydrated patients. ACE inhibitors, ARBs, and other blood pressure medicines can add to its blood-pressure-lowering action. Combining it with other diuretics, corticosteroids, stimulant laxatives, or certain asthma medicines may deepen potassium loss or other electrolyte disturbances.
Aminoglycoside antibiotics and other medicines that can affect hearing or the kidneys may present added risk when used with furosemide. Lithium levels can rise because furosemide may reduce lithium clearance, while low potassium or magnesium can increase susceptibility to digoxin toxicity. Sucralfate can reduce furosemide absorption and is generally separated from oral furosemide by at least two hours. Bile acid sequestrants may also reduce absorption, and phenytoin may weaken the diuretic response. High-dose salicylates, cisplatin, cyclosporine, and methotrexate are among the other medicines that may require specific assessment.
Do not stop an important medicine or adjust its timing without professional guidance. Give your prescriber and pharmacist an up-to-date list of all medicines, vitamins, herbal products, electrolyte supplements, and nonprescription pain relievers. Potassium products and salt substitutes containing potassium should be used only as directed because the need for supplementation varies with kidney function, diet, laboratory results, and other medicines.
After Using Lasix
Storage
Store Lasix tablets at 68°F to 77°F; excursions from 59°F to 86°F are permitted. Keep the tablets in their original, tightly closed container and protect them from light. Avoid humid storage areas such as a bathroom cabinet, and keep the medicine out of reach of children and pets. Do not use tablets that are damaged, contaminated, or beyond their expiration date.
Disposal
Do not keep unused or expired Lasix tablets indefinitely or give them to another person. The preferred disposal route is a community drug take-back program or an authorized collection kiosk, including periodic national take-back events. These services reduce the risk of accidental ingestion and prevent personal medicine from being used by someone for whom it was not prescribed.
If no take-back option is available, follow local household disposal instructions. In general, tablets can be removed from their container, mixed without crushing with an unpalatable substance such as used coffee grounds or kitty litter, placed in a sealed bag or container, and discarded in household trash. Remove or obscure personal information on the original label before recycling or discarding the empty container. Do not flush furosemide tablets unless a current official disposal instruction specifically directs you to do so.
Alternatives to Lasix Tablets
Furosemide itself is also available from us as generic Furosemide tablets, providing the same active ingredient in a different presentation. A switch between products should still preserve the prescribed dose and be checked when tablet strength, inactive ingredients, or appearance changes.
Other loop diuretics include torsemide and bumetanide. They act at a similar kidney site but differ from furosemide in potency, absorption, and duration, so their doses are not interchangeable milligram for milligram. Ethacrynic acid is a non-sulfonamide loop diuretic that may be considered when sulfonamide-related structure is a particular concern, although it has its own adverse-effect and monitoring considerations.
For hypertension or milder fluid retention, thiazide-type diuretics such as Hydrochlorothiazide may be considered. Thiazide-like medicines such as chlorthalidone or indapamide have related therapeutic roles but different durations and dosing. Potassium-sparing diuretics such as spironolactone are sometimes combined with a loop diuretic rather than used as a direct substitute. Such combinations can help in selected conditions but may also cause excessive diuresis or abnormal potassium levels.
The most appropriate option depends on the reason for treatment, kidney and liver function, blood pressure, electrolyte results, allergies, and other medicines.
