How Ampicillin Works

Without a properly assembled wall, actively dividing bacterial cells become structurally unstable and eventually rupture.

This mechanism is shared with other beta-lactam antibiotics. Ampicillin's chemical structure extends its activity to certain gram-negative organisms that are not adequately covered by narrower-spectrum penicillins. However, unprotected ampicillin can be broken down by bacterial beta-lactamase enzymes, so it is not reliable against beta-lactamase-producing strains unless testing establishes susceptibility.

Ampicillin undergoes little metabolism and is eliminated largely unchanged, primarily through the kidneys by glomerular filtration and active tubular secretion, with a smaller amount excreted in bile. Its elimination half-life in adults with normal kidney function is approximately one to one and a half hours. The half-life becomes longer when kidney function is reduced, which is why dose or interval adjustments may be necessary.

Important Safety Information

Ampicillin is contraindicated in anyone with a previous serious hypersensitivity reaction to ampicillin or another penicillin. Side Effects and When to Get Help

The most common effects of oral ampicillin involve the digestive system and may include nausea, loose stools, diarrhea, and stomach discomfort. Oral or vaginal yeast overgrowth can occur because the antibiotic also reduces normal protective bacteria. Headache and mild skin eruptions are less common. Report a new rash to the prescriber rather than deciding independently that it is harmless or restarting the medicine after it has been stopped.

Seek emergency care immediately, including calling 911 or going to the nearest emergency department, for facial, lip, tongue, or throat swelling; difficulty breathing or swallowing; faintness; widespread hives; or a severe blistering or peeling skin reaction. Serious allergic reactions can occur even in someone who has taken a penicillin before without difficulty.

Watery or bloody diarrhea that is severe, persistent, or accompanied by fever or abdominal cramping requires prompt medical evaluation. Antibiotics can occasionally trigger Clostridioides difficile-associated diarrhea and colitis during treatment or after the course has ended. Do not try to manage suspected antibiotic-associated colitis solely with an over-the-counter antidiarrheal unless a clinician advises it. Unusual bruising, marked fatigue, jaundice, reduced urination, confusion, or seizures also warrant prompt assessment.

Overdose and Accidental Exposure

Taking more ampicillin than prescribed can increase nausea, vomiting, diarrhea, and other adverse effects. Very high exposure, especially when kidney function is impaired, can cause neurotoxicity, including confusion, muscle twitching, or seizures. The likely effects depend on the amount taken, the person's age and kidney function, and any other medicines involved.

Call 911 for collapse, a seizure, breathing difficulty, or inability to wake. Do not induce vomiting unless a medical professional specifically instructs you to do so. Ampicillin is removed to some extent by hemodialysis, which may be considered by clinicians in a severe overdose, particularly when renal function is poor.

Storage

Store ampicillin capsules at 68°F to 77°F in a tightly closed container protected from moisture. Keep them away from direct sunlight, excessive heat, bathrooms, sinks, and other damp locations. Leave the capsules in the labeled dispensing container unless a pharmacist provides different storage instructions, and keep them out of the sight and reach of children and pets.
